How To Enroll
1) Email your interest to Synergyinserviceindusty@gmail.com by clicking on the button above
2) Provide proof of employment in a Chaffee County restaurant
3) Synergy will send you a waiver to sign and a short screening to complete
4) You will receive an ID member number and a list of providers to choose from. Email synergy if you want help finding the right provider, we are happy to help.
5) Schedule directly with the provider using your ID number and the provider will bill Synergy for the first 6 sessions.

You will choose one of our offered Providers. Once you are enrolled with Synergy and have an ID #, schedule an intake appointment with your Provider of choice directly. You will fill out the Provider’s paperwork and Synergy will be billed by the provider.

You will work with Solvista as a provider. They will bill your Medicaid plan. If you need to sign up for Medicaid or want to see if you could be eligible for benefits scroll down to the next section on this page.

Providers will need to bill your health insurance. Synergy can help support enrollment processes and pay for copays if you have private insurance. You will work with one of our Private Providers.

Synergy will cover 6 sessions with your provider. The provider is able to recommend to Synergy that additional services are needed after the initial 6 sessions have concluded and a review will be done by the Synergy Selection Committee to determine extended benefits.
